The State of Arizona does not issue Single Status Certificates. You will need to prepare a Single Status Affidavit that will be signed by you and notarized by an Arizona notary public. An Affidavit of Single Status is a sworn statement in which you attest that you are single and free to marry. We recommend that you ask you fiance(e) to find out if your affidavit needs to be in any specific form or contain some specific additional details. The requirements differ from country to country.

Your Single Status Affidavit must be properly notarized. Ensure the notary includes either an acknowledgment or a jurat. Documents that only contain the notary’s signature and stamp, without the proper notarial certificate, will be rejected. Remote online notarizations are also eligible for apostilles, but the notarial statement must include the following language: "This remote online notarization involved the use of communication technology."
