Arizona apostille requirements for notarized documents
The documents must be properly notarized. Remote online notarizations are acceptable for apostilles, provided the notarial certificate includes the required statement: "This remote online notarization involved the use of communication technology."
If you need to obtain an Arizona State apostille for a power of attorney, authorization, consent, or affidavit notarized by an Arizona notary public, ensure that the notary includes either an acknowledgment or a jurat. Documents that only have a notary’s signature and stamp — without the notarial certificate — will be rejected.
Below are sample certificates for use in Arizona (either one may be used):
Certificate of Acknowledgment:
(for personal and corporate powers of attorney, affidavits, statements, letters, consents, certified diplomas and transcripts signed by school officials, etc.)
Jurat:
