How do you prepare an Arizona high school, college, university transcript for an apostille?

importantAcademic documents such as diplomas, transcripts, verification letters, etc. issued by high schools, colleges and universities of Arizona, must be certified by the school registrar, and notarized prior to be presented for an apostille. A certified copy must have the original signature of the school official.

Sample certification:

This is the original transcript of [ name of the student ].
[ Original signature of the school official ] [ Date ]

importantThe signature of the school official must be properly notarized by an Arizona notary. The notary should use a Certificate of Acknowledgment ot Jurat (Arizona Notary Public Reference Manual

Certificate of Acknowledgment:

State of Arizona         )

County of _________)

On this ___ day of ___________, 20__, before me personally appeared    (name of the school official who signed the document)  , whose identity was proven to me on the basis of satisfactory evidence to be the person who he or she claims to be, and acknowledged that he or she signed the above/attached document.

(seal)                                        (notary signature)

                                                 Notary Public

Other acceptable notarial certificates:

Jurat:

State of Arizona     )
County of . . . . . . .    )

Subscribed and sworn to before me this ........ day of ........, 20 ...... by (name of the school official who signed the document)

(seal)                                         (notary signature)

                                                 Notary Public

Can a notary use a separate page for acknowledgment / jurat?

question answerA notary may use an additional sheet of paper to attach the notarial certificate to a document if there is no room left on the document. If the notary attaches a certificate, the notary should title the page with the words “Notarial Acknowledgement”, “Verification on Oath or Affirmation (Jurat)”, or Copy Certification. In order to prevent someone from affixing the attached certificate to a different document, the notary must also describe the attached document on the page containing the notarial certificate in as great of detail as possible.

Can a notary certify a copy of a transcript?

question answerYes, the State of Arizona allows copy certification by notaries public. However, please check with the receiving foreign school / agency if they will accept a notarized copy. Many foreign schools require an original transcript signed by the school principal or registrar.

In case a notarized copy is acceptable, the notary should use the following notarial certificate (Arizona Notary Public Reference Manual):

Copy Certification:

State of Arizona      )
County of . . . . . . .    )

I,   (name of the notary)  , a notary public, do certify that, on  the __ day of _________, 20__, I personally made the above/attached copy of   (Document Title)   from the original, and it is a true, exact, complete, and unaltered copy.

(seal)                                         (notary signature)
                                                 Notary Public
